Основа для індивідуалізації підходів до хіміотерапії у хворих на ранній стадії раку грудної залози: біологічні чи анатомічні характеристики пухлини?

Традиційні критерії визначення необхідності застосування ад'ювантної хіміотерапії

Мета-аналіз The Early Breast Cancer Trialists’ Collaborative Group (EBCTCG) продемонстрував істотні докази ефективності ад'ювантної хіміотерапії при ранній стадії раку грудної залози [6,7]. В останньому звіті, опублікованому у 2005 році за даними 60 рандомізованих досліджень (розпочатих до 1995 року), проведено порівняння груп пацієнтів, що отримували хіміотерапію, і тих, що її не отримували (29 000 випадків із яких 10 000 були летальними) [7].
За 15-річний період спостереження зареєстроване значне скорочення абсолютного ризику рецидивів і рівня смертності в групах хворих на рак грудної залози, які отримували хіміотерапію у порівнянні з аналогічними групами пацієнтів без хіміотерапевтичного лікування.
Серед жінок віком молодше 50 років, застосування хіміотерапії скоротило ризик виникнення рецидиву та рівень смертності на 37% і 30% відповідно, а абсолютний приріст рівня виживаності зріс удвічі у показниках як 15-річної, так і 5-річної (10% проти 4.7%). Серед літніх жінок (50-69 років) за період 15-річного спостереження річний рівень рецидиву та смертності зменшився на 19% та 12% відповідно, що у перекладі на абсолютний приріст складає 4,1% і 3% відповідно [7,8]. Таке відносне пропорційне скорочення обох показників (рецидивів і смертності від раку молочної залози) було майже однаковим у вузол-негативних і вузол-позитивних пацієнтів, а всі абсолютні показники були явно кращими у вузол-позитивних хворих.
Крім того, було продемонстровано, що найбільший ефект від ад'ювантної хіміотерапії відбувається в перші кілька років після проведення цієї терапії. Додатковий аналіз підгруп показав, що ад'ювантна хіміотерапія була ефективна як в групах пацієнтів з естроген-негативними, так і з естроген-позитивними пухлинами, хоча більш кращі результати спостерігалися саме в першій групі пацієнтів. Дані багатоцентрових досліджень Cancer and Leukemia Group B (CALGB), що проводились серед жінок з позитивним статусом лімфатичних вузлів при раку грудної залози виявили, що естроген-рецептор-негативні пухлини демонструють кращу відповідь на хіміотерапію у порівнянні з естроген-позитивними пухлинами [7,9]. Істотне скорочення абсолютного рівня рецидивів та смертності на 12,3 та 9,2% спостерігалося у групі пацієнтів віком до 50 років, та на 8,6 і 6,1% у пацієнтах віком 50-69 років відповідно. Загалом, рішення щодо призначення ад'ювантної хіміотерапії засновується на прогнозуванні ризику рецидиву захворювання і на оцінці ймовірної користі від лікування. Для визначення необхідності застосування ад'ювантної хіміотерапії часто використовується показник порогу 10% ризику виникнення віддалених рецидивів [10]. Виділяють наступні клініко-патологічні прогностичні фактори виживаності для пацієнтів з ранньою стадією раку грудної залози: розмір новоутворення, статус лімфатичних вузлів, гістологічний тип, естроген- і прогестерон- позитивність або негативність, HER-2-експресія та лімфоваскулярна інвазія. Деякі емпірично обґрунтовані та обумовлені значною доказовою базою міжнародні рекомендації по лікуванню [3,4,11,12] надають рекомендації по застосуванню ад'ювантної хіміотерапії на ранніх стадіях раку грудної залози. Наприклад, рекомендації St. Gallen [4] стверджують, що критеріями, засновуючись на яких, можна не призначати ад’ювантну хіміотерапію хворим з ранньою стадією молочної залози є: розмір пухлини менше 2 см, 1 стадія захворювання, естроген- і прогестрон-позитивний тип пухлини, HER2-негативний статус, вік старше 35 років та відсутність судинної інвазії. Всі інші пацієнти, що не відповідають даним критеріям, мають отримувати ад'ювантну хіміотерапію, адже ризик віддалених рецидивів в таких випадках складає більше 10%. Adjuvant! Online – це незалежно підтверджений веб-ресурс, що широко використовується для оцінки показників виживаності та ризику виникнення рецидиву. Він поєднує у собі численні клінічні та морфологічні фактори і може використовуватися для оцінки вищеозначених критеріїв у пацієнтів з ранньою стадією раку грудної залози. Також цей ресурс здатен враховати вплив супутніх захворювань при визначенні прогнозу та давати оцінку різним терапевтичним стратегіям [13]. Однак деякі дані говорять про те, що отримані результати в цілому дещо завищують прогноз, і також при цьому не враховуються такі важливі елементи, як, наприклад, HER2-статус. Рекомендації по лікуванню, що описані вище, та Adjuvant! Online не можуть виявити значну гетерогенність, що існує у пацієнтів з однаковою стадією та типом пухлини. Кілька незалежних дослідницьких груп провели комплексний аналіз профілювання експресії генів, використовуючи технологію мікрочіпів, з метою розробки нових, більш ефективних, методів, які допоможуть оцінити риск рецидиву, що допоможе при прийнятті рішень щодо застосування ад'ювантної хіміотерапії.
