Рекомендації щодо лікування пухлин з імуногістохімічно підтвердженою наявністю рецепторів епідермального фактора росту (EGFR) є наступні:

• Цисплатин 80 мг/м2 в/в на день 1 плюс вінорельбін 25 мг/м2 в/в на 1 та 8 дні плюс цетуксимаб 400 мг/м2 - внутрішньовенна навантажувальна доза, потім 250 мг/м2 в/в щотижнево кожні 21 день (продовжити цетуксимаб щотижня після закінчення чотирьох-шести циклів або до прогресування захворювання). [45, 46]

Ерлотініб, афатініб та гефітиніб затверджені Управлінням з санітарного нагляду за якістю харчових продуктів і медикаментів США (FDA) для первинної терапії метастатичного НДКРЛ у пацієнтів, де пухлина має делеції EGFR exon 19 або мутації заміщення екзону 21 (L858R), та якщо це було виявлено FDA - схваленим тестом: cobas EGFR mutation test та EGFR RGQ PCR Kit. Крім того, афатиніб призначений для терапії першої лінії при метастатичному НДКРЛ для 3 додаткових нерезистентних EGFR-мутацій (тобто L861Q, G719X, S768I). Рекомендації щодо лікування включають в себе наступне:

• Ерлотініб 150 мг per os щодня до прогресування захворювання. [7, 30, 49, 50, 51, 52, 53, 54, 55]
• Афатініб 40 мг per os щодня до прогресування захворювання.[56]
• Гефітиніб 250 мг per os щодня до прогресування захворювання.[57, 58]

Рекомендації щодо лікування ALK – позитивної (кіназа анапластичної лімфоми) місцево поширеної або метастатичної пухлини є такими:

• Крізотініб 250 мг per os 2 рази на добу до прогресування захворювання; переривання дозування та/або зменшення дози до 200 мг 2 рази на добу per os може вимагатись, виходячи з безпечності та переносимості; зниження дози до 250 мг на добу – якщо потребує клінічна ситуація [59]
• Серітініб 750 мг per os щодня до прогресування захворювання; переривання та/або зменшення дози можуть вимагатись з огляду на безпечність та переносимість. [60]

Резистентнісь/непереносимість кризотиніба:

• Алектиніб 600 мг 2 рази на добу per os до прогресування захворювання; переривання та/або зменшення дози можуть вимагатись з огляду на безпечність та переносимість. [61]

Пембролізумаб може бути використаний як монотерапія першого ряду для пухлин із високою експресією PD-L1 [бал пропорції пухлини (TPS) ≥50%)], як визначено затвердженим FDA тестом, без EGFR або ALK-геномної аберації за наступними схемами: [62]

• Пембролізумаб 200 мг в/в кожні 3 тижні до прогресування захворювання або неприйнятної токсичності або до 24 місяців у пацієнтів без прогресування захворювання.

Пемпролізумаб також вказаний у комбінації з пеметрекседом та карбоплатином як терапія першої лінії у пацієнтів з метастатичним неплоскоклітинним НДКРЛ, незалежно від експресії PD-L1:[62]

Пембролізумаб 200 мг в/в плюс пеметрексед 500 мг/м2 плюс карбоплатин (AUC 5 мг/мл/хв) в/в на 1-й день кожного 21-денного циклу на 4 цикли, потім пембролізумаб 200 мг в/в кожні 3 тижні до прогресування захворювання або неприйнятної токсичності або до 24 місяців у пацієнтів без прогресування хвороби.

Рекомендації для пацієнтів з наявністю протипоказань щодо використання карбоплатину або цисплатину:

• Гемцитабін 1100 мг/м2 в/в на 1 та 8 дні плюс доцетаксел 100 мг/м2 в/в на 8 день кожні 21 день [63, 64]
• Гемцитабін 1000-1200 мг/м2 в/в на 1 та 8 дні плюс вінорельбін 25-30 мг/м2 в/в на 1 та 8 дні кожні 21 день. [38, 65, 66, 67]

Друга лінія хіміотерапії (метастатична або рецидивна форма захворювання)

IV стадія або рецидивна форма захворювання

Друга лінія хіміотерапії актуальна у випадках поширеної або рецидивуючої форми хвороби у разі прогресування захворювання після першої лінії терапії. Схема другої лінії хіміотерапії:

• Ніволумаб 3 мг/кг в/в кожні 2 тижні до прогресування захворювання або прояв неприйнятної токсичності [68]
• Пембролізумаб 200 мг в/в кожні 3 тижні до прогресування захворювання або прояв неприйнятної токсичності (до 24 місяців) у пацієнтів з PD-L1 позитивними пухлинами. Пацієнти з наявністю EGFR або ALK геномних аберацій повинні проходити курс терапії за схемою, схваленою FDA для цих аберацій, до прийому пембролізумабу [69]
• Доцетаксел 75 мг/м2 в/в на 1 день кожні 21 день (ціль - 4-6 циклів)[70, 71, 72, 73, 74] +/- рамуцирумаб 10 мг/кг в/в [75]
Пеметрексед 500 мг/м2 в/в на 1 день (неплоскоклітинна гістологія) кожні 21 день (ціль - від чотирьох до шести циклів: включають препарати фолієвої кислоти та вітаміну B12 разом із премедикацією дексаметазоном для пеметрекседу) [74]
• Ерлотиніб у дозі 150 мг на добу per os для пацієнтів з наявністю EGFR-мутацій або ампліфікацій до прогресування захворювання. [7, 30, 49, 50, 51, 52, 53, 54, 55]
• Афатиніб 40 мг per os щоденно для пацієнтів з метастатичним плоскоклітинним НДКРЛ на фоні прогресії після проведення хіміотерапії на основі препаратів платини.[76]
• Дослідження Herbst et al підтверджує, що бевацизумаб та ерлотиніб не повинні використовуватися разом у випадках рефрактерних або рецидивуючих форм НДКРЛ; монотерапія Ерлотинібом у якості другої та третьої ліній терапії залишається принятим стандартом.[77]
